Instructions for Previous Coursework Form
• Applicants should fill out the previous coursework form as completely as possible.
• The form should be sent to SESE at sese_admit@asu.edu with the subject line “Academic Record for YOUR NAME”.
• If you are unable to fill out the form electronically, we will accept one filled out by hand.
• If you are unable to email the form, you may send it via mail or fax to SESE. Our address is:
